This comprehensive, preventive health assessment is designed to detect early warning signs of chronic disease — and help delay or even prevent its onset. Your doctor will assess your lifestyle, medical and family history, and current health status to determine any risk factors and guide you on the best steps forward.
Together, you and your GP can create a plan to support your long-term wellbeing.
Am I Eligible?
You may be eligible for this bulk-billed health check if you:

Are aged between 45 and 49 years

Have at least one of the following risk factors:

Annual Health Assessment
Annual Health Check for Patients Aged 75 and Over
If you’re 75 years or older and living at home, you are eligible for a bulk-billed annual health assessment — meaning there is no cost to you. This yearly check-up provides a comprehensive overview of your current health and wellbeing, helping you to maintain independence, stay active in your community, and access any support you may need.
Why Is This Health Check Important?
The purpose of the annual assessment is to identify any risk factors or early signs of health concerns that may benefit from further attention. It also considers the broader aspects of your physical, emotional, and social wellbeing.
This holistic approach ensures your care plan supports all aspects of your life, allowing you to remain healthy, independent, and connected.
What to Expect
Your health assessment will be conducted by one of our experienced and friendly nurses in a confidential and comfortable setting. It includes:

Relevant examinations and investigations

A full review of your health status

Personalised advice and recommendations

A written summary of your health check, including advice for ongoing care

A copy of the report can also be shared with your carer (if appropriate and with your consent)
During your appointment, the following components will be assessed:

Immunisation status, including influenza, tetanus, and pneumococcal vaccines

Physical function – your mobility, daily activities, and any recent falls

Mental and cognitive health – mood, memory, and psychological wellbeing
